Does the fee apply if I have zero stock at all?

No. The fee is charged per unit shipped, so if a product has completely sold out, there's nothing to charge the fee against. That's not really a way to "avoid" it in any useful sense, though, since a stockout costs you far more in lost sales and ranking.

Does the fee apply to FBM orders?

No. It's specific to FBA-fulfilled units. Orders you fulfill yourself aren't subject to it.

Will restocking mid-week fix my days-of-supply number immediately?

Not necessarily. The historical days of supply metric refreshes on a weekly cycle, so there can be a lag between a shipment arriving and it showing up in Amazon's calculation.

Do inbound shipments count toward my inventory for this calculation?

No. Amazon only counts sellable FBA inventory that's already been received into a fulfillment center. Units still in transit don't help your days-of-supply number until they're fully checked in.
